LAIS Forms

• Plan of Study - Outlines all remaining requirements for the degree along with a statement of purpose.

• Qualified Individual Form - LAIS applicants must present their proposed degree plan to at least one qualified individual teaching or working in the field related to the student's professional goals.

• Career Development Facilitator Form - LAIS applicants must also present their proposed degree plan to a Career Development Facilitator through UNM's Career Services Office.
